Continuing Education Provider Policies

1)    Confidentiality 
a.    All information collected during registration (Including name, address, telephone number, payment information, and email address) will be kept confidential. 
b.    Participants will not be recorded without their written permission. 
c.    Participants will be notified by email/phone promptly if there is a confidentiality breach. 
2)    Complaints 
a.    If a participant or potential participant would like to express a concern about the CE provider or program, the individual may call 773-217-0159 or email carriewhite@kaleidoscopecounselingil.com. Although I not guarantee a particular outcome, I will consider the complaint, make any necessary decisions, and respond within 48 business hours. 
3)    Fees, Refunds, Cancellations 
a.    Fees will be stated on the website and in advertising materials. Fees are non negotiable unless you believe you qualify for a sliding scale due to hardship. 
b.    A refund will not be issued after the presentation date has passed. 
c.    If you need to cancel, you may do so for a full refund up to 24 hours in advance. 
4)    Attendance 
a.    If participant is not able to attend the full session partial credit may be given if there are extenuating circumstances that happen after the program has already begun. 
5)    Program Materials 
a.    Materials may be updated if there is any relevant new information. 
6)    Disclosure of Client Information
a.    No specific client information will be disclosed unless said client has given informed consent. 
7)    ADA Accommodations 
a.    All reasonable requests for accommodations will be honored to the best of my ability. If I am unable to provide accommodations you will be issued a refund.
